Started in 1969 as a family planning clinic, Bronx Health Center joined Community Healthcare Network (CHN) in 1981 and expanded its services to become a comprehensive health center in the late 1990’s. Since then, this center has been providing a diverse patient population services that include:
Primary medical care; a Transgender Program; a Diabetes Management Program accredited by the American Diabetes Association; family planning and reproductive health care; health education; HIV primary care, case management, counseling and testing, and support services, immunizations; nutritional services; pediatric care; prenatal and postpartum care; school physicals; social services; Access to Healthcare Program, which helps patients enroll into the NYS AIDS Drug Assistance Program; asthma screening and care; birth control and safe sex education; Emergency Contraception; and the Treatment Adherence Program, which provides support to patients to help them follow their medical regimens.
A financial representative works with all patients to choose payment options. As a Federally Qualified Health Center, CHN does not turn away any patient under any circumstances.
The center primarily serves residents in the neighborhoods of Mott Haven, Hunts Point and Morissania. taking appointments from 9 AM- 7 PM on Monday, 9 AM-5 PM on Tuesday and Thursday, 9 AM-6PM on Wednesday, 9 AM-12:45 PM on Friday, and 9 AM-3:30 PM on Saturday. By subway: #2 or #5 trains to Intervale Avenue, walk 1 1/2 blocks to Center.
By Bus: #BX-4 to Westchester and Tiffany Avenues, walk across the street to Center.
