Notice of Data Incident

Thursday, November 30, 2023 – Community Healthcare Network, Inc. (“CHN”) provides primary healthcare and support services to individuals from a diverse range of communities in New York City. It is announcing a data privacy incident that may have impacted individuals’ information.

On October 1, 2023, CHN experienced a major network disruption that affected CHN’s ability to access certain systems. Upon discovery, CHN immediately engaged cybersecurity specialists to assist in the incident response. This included recovering impacted systems and quickly returning to normal business operations. Additionally, a forensic investigation, which included expert third-party specialists, was commenced to determine the nature and scope of the incident. CHN notified law enforcement officials and is cooperating with their investigation.

As part of the investigation, CHN confirmed that certain information was accessible as a result of the incident and undertook a thorough and time intensive review to determine the individuals and associated information potentially affected. On July 21, 2024, this investigation was completed and the types of information identified to be affected included individuals’ names along with addresses, phone numbers, dates of birth, health insurance information, health insurance ID number, patient ID number, provider name, service date, Social Security numbers, financial account information, driver’s license/state identification number, medical record number, prescription information and/or medical history information. CHN is directly notifying individuals whose information is identified to have been impacted where possible.

As an added precaution, CHN is offering potentially impacted individuals free access to credit monitoring and identity protection services. If individuals have questions about this incident, or would like to enroll in the credit monitoring and identity protection services, they may contact a dedicated assistance line which can be reached at 877-881-0623 Monday through Friday, 9 a.m. to 9 p.m. Eastern Time (excluding major U.S. holidays).

Individuals are encouraged to remain vigilant against incidents of identity theft by regularly reviewing account statements and credit reports to look for mistakes or fraud.  Individuals can get one free credit report every year from each of the three major credit reporting bureaus: TransUnion; Experian; and Equifax. To order a free credit report, individuals may visit www.annualcreditreport.com or call 1-877-322-8228.

Individuals can further educate themselves regarding identity theft, fraud alerts, credit freezes, and steps to protect their personal information by contacting the credit reporting bureaus, the Federal Trade Commission (“FTC”), or their state Attorney General. Instances of known or suspected identity theft should also be reported to law enforcement, the state Attorney General, and the FTC.

What does the Notice of Data Incident mean?

On October 1, 2023, Community Healthcare Network’s (CHN) online system was attacked. As soon as CHN learned of the problem we acted right away. CHN hired specialists who quickly fixed the problem and returned the systems to normal. CHN, cyber-security specialists, and the law enforcement are working to learn more about the attack.

During this attack, some patient information was impacted. This information varies by individual, but might include names, addresses, phone numbers, dates of birth, health insurance information, health insurance ID number, patient ID number, provider name, service date, Social Security numbers, financial account information, driver’s license/state identification number, medical record number, prescription information and/or medical history information. CHN is sending notice letters to identified individuals by mail where possible.
