Care for Every Student

Your child’s health is important, and now it’s easier to keep them healthy with our School-Based Programs.

School-Based Health Centers (SBHC) are full-service healthcare centers found in schools. They offer the same care as your local doctor’s office, without the trouble of missing school or work. We take great pride in being able to offer broad healthcare
services to our patients.

Services we provide to keep your child healthy:

  • Check-ups for school, sports, and work papers
  • Counseling (one-on-one, group, family)
  • Shots and flu shots
  • Eyesight, hearing, and nutrition screenings

  • Routine care of long-term health problems, such as
    asthma and diabetes (high blood sugar)
  • Education on healthy eating and weight control
    programs
  • Reproductive Healthcare (Birth Control, Pregnancy, and STI Testing)

How to signup:


  • Complete the Consent Form
    • Pick up a consent form at the SBHC office, school, or download it below.
    • Fill it out with your child’s medical information, insurance, and contact details.
  • Submit the Consent Form
    • Drop off the completed form at the SBHC office, school main office, send it via email, or mail. SBHC Staff will register the patient.
  • Schedule an Appointment
    • The SBHC team will help students and parents or guardians book an appointment with the provider.
  • Attend the Appointment
    • Students will attend their SBHC at the scheduled time. Parents are welcome to attend but not required.
  • Ongoing Care
    • The SBHC will provide follow-up care, including check-ups and any needed treatments.

  • The program takes care of both physical and mental health in one place, so your child gets the support they need throughout school. Plus, you only need to enroll once, and your child will be covered until they graduate.
  • Your child can keep their doctor even when they sign up for our program.
  • Parents do not need to be at School-Based Health Center visits. One of the helpful parts of having an SBHC at the school is that
    you don’t have to miss work for your child’s doctor visits.
  • Immigration status or lack of health insurance will not affect a student getting healthcare at SBHC.

School Immunizations

New York State law says students must get certain vaccines to go to school. These rules have changed for the 2024-2025 school year.
